38 Perkin was a former WHO employee who now ran a non-profit in Seattle – PATH – and quickly became Gates’ go-to-guy on all things global health. At a dinner on the top floor of a Seattle downtown hotel, Perkin had Gates trying a new syringe that PATH had developed for single use (to avoid re-uses and thus the potential spread of diseases), injecting saline solution into a bread roll. Not long after, Gates made a $100 million grant to PATH that would see it attempt to make common childhood vaccines more readily available.

44 The strong crowding-in effect from the foundation’s emphasis on global health will be greatly felt in the state of Washington where it resides. More than 12,000 people are now employed there under the broad umbrella of global health, across research, logistics and manufacturing, an industry amounting to $6 billion annually.

46 In sheer numbers, global development is actually the largest division in the foundation with $2,211,000 spent in 2016, compared with $1,197,000 in the global health division. But this balance is mainly so because delivery-related departments from global health have been moved into global development (as described earlier in this chapter), such as polio and vaccine delivery, combined representing 42 per cent of the entire global development budget.
